We provide support alongside individuals and families in transitioning into SDP by which individuals control their own lives, make their own choices, and take responsibility for their actions, particularly in personal goals, independence, and self-advocacy. (SDP Service codes) 

1. Collaborating with individuals and their support networks to identify needs, goals, and desired services.

2. Assisting in developing person-centered plans to ensure alignment with participant aspirations and capabilities.

1. Accessible Housing: Ensure that housing options are accessible, including features like lowered kitchen counters, widened doorways, and wheel-in showers.

2. Affordable Housing: Provide affordable housing options to prevent institutionalization or homelessness.

3. Advocacy and Rights Protection: Advocate for the rights of individuals with developmental disabilities and ensure compliance with the Fair Housing Act.

4. Community Integration: Promote community integration and independent living by providing housing within the community.

5. Collaboration with Agencies: Work with state and local agencies to develop and subsidize rental housing.

6. Resource Referral: Provide information and referrals to appropriate resources and agencies.

7. Independent Living Skills Training: Assist consumers in achieving their independent living goals through skills training.

8. Housing Assistance: Help consumers with housing-related services, including finding and maintaining housing.

We provide personalized guidance, support, and resources to individuals with disabilities and their families. Through tailored strategies and compassionate support, we empower participants to build confidence, develop essential skills, and foster a sense of independence and fulfillment.

1. Providing ongoing coaching and mentorship to empower participants in decision-making and goal achievement.

2. Supporting Parent/Consumer 1:1/ Small Group Coaching supporting self-advocacy skills to navigate systems.

3. Community Integration Coach (SDP Service Code 331): Support for active community participation, building social connections, and accessing local resources.

4. Employment Coach (SDP Service Code 335): Assistance with finding, securing, and maintaining employment, including job coaching, workplace accommodations, and skills training.

5. Life Coach: Tailored services to meet individual needs and goals, ensuring a fulfilling and independent life. Individual Training & Education (SDP Service Code 334)

We provide Independent Facilitator services, where a dedicated facilitator, chosen and directed by the participant, assists in planning and accessing the necessary services to implement the participant’s Individual Program Plan (IPP). IF (SDP Service Code 340)

1. Establishing effective communication channels between regional centers.

2. Facilitating seamless coordination to enhance service delivery and participant satisfaction.

 We provide hands-on assistance and care to individuals with disabilities to help them with daily living activities, personal care, and community participation, ensuring they can lead independent and fulfilling lives. Community Living Support (SDP Service Code 320 for individual or agency support and 321 for Residential facilities) 

 1. Independent Living Skills Training: Assist consumers in achieving their independent living goals through skills training.

Inclusion Advocacy services offer dedicated support to create environments where everyone feels a sense of belonging and empowerment. Our services focus on challenging systemic barriers, biases, and prejudices that hinder the participation and advancement of marginalized groups, including people with disabilities. Community Integration Support (SDP Service Code 331)

1. Advocacy: Advocating alongside participants to ensure their voices are heard and their choices are respected.

2. Information & Referral: Offer information and referrals to appropriate resources and agencies.

3. Community Collaboration: Develop and maintain relationships with community organizations to expand community connections and inclusion opportunities.
